APPOINTMENTS
New Patients
PLEASE NOTE: Currently, I am NOT accepting any patients with Medicaid, Medicare, or any other governmental insurance. Patients with commercial health insurance through an employer or the Affordable Care Act ('Obamacare') may apply for an initial consultation, and their plans should cover their medications, but appointments with me will be self-pay. However, you very well might be able to get reimbursed for the cost by your insurance if I provide you with a medical receipt. Please see Services/Fees for details.
1) Please click here: New Patient Screening Form to fill out and submit. I will typically respond within 24 hours (excluding weekend or holidays). PLEASE NOTE: some patients have reported having difficulties sending the form or receiving a reply when using Safari, Internet Explorer, or mobile devices. If you do not receive a confirmation within 24 hours, please use a different browser (e.g., Chrome or Firefox) or call the office at 814-823-5640 to schedule. Currently, I should be able to schedule you for an appointment WITHIN 7 DAYS (depending upon your availability).
2) Once I notify you that I am able to meet your needs as a new patient, please click here: SCHEDULE to search for and select an available appointment date and time (you should receive electronic confirmation of your appointment prior to assuming it has been confirmed). To ensure I have time to respond to your request please schedule for the NEXT DAY at soonest. If you cannot find anything with 7 days, feel free to contact me via my phone number or the email I have sent you, and I will do my best to schedule your initial appointment within 7 days, depending upon your availability.
3) Prior to your Initial Appointment, please click here: New Patient History Form to fill out and submit (this can be done on many mobile devices as well). This will save you from having to fill out forms on the day of your appointment.
Simply bring a valid form of ID and a form of payment (cash/credit/debit/HSA card) to your appointment. I will see you there! If you have had any difficulty with the above steps please feel free to simply call 814-823-5640 and I will gladly assist you in setting up an appointment. At your initial appointment, we will also set up a Patient Fusion portal account (don't sign up for one before your appointment though, because it will not be linked to my practice), so that you can message me directly regarding any aspect of your treatment (I do not use an administrative assistant or medical assistants: your questions are always read directly by myself and I will always be the one who responds to you). I will respond to your questions the day you send them, unless it is after business hours, in which case I will respond to you the next day. There is no additional charge for portal messaging (it is all included). You will be able to have access to my psychiatric advice whenever you need it.
